Insurance Reimbursement Due to the high cost of processing insurance MIDLINE does not accept health insurance. We require payment at the time of your treatment BUT we will provide the necessary information and do everything we can to help you obtain reimbursement from your insurance company. You must have a PPO or similar insurance plan to be eligible for reimbursement. You should also see your doctor (M.D., Chiropractor or Osteopath) and have him/her provide you with a diagnosis and icd-9 code (a number that corresponds to the diagnosis). It is also important to check directly with your insurance company to see if they cover Massage, Soft-Tissue Mobilization and Myofascial Release Therapy.
We will provide you with a Superbill for each appointment that will contain all the required information your insurance company will need. Patients who have PPO’s and similar plans regularly receive coverage for our services. |
